چکیده
Volume 95 No. 7 July 2012 a previously healthy 20 year-old male presented to the Rhode Island Hospital Emergency Department with 24 hours of worsening abdominal pain, vomiting, and fevers. On physical examination he appeared diaphoretic and febrile, with an oral temperature of 104.3°. The patient was noted to have tenderness to palpation in the epigastric region. Initial laboratory evaluation revealed a white blood cell count of 23,000/μl. A CT scan of the abdomen and pelvis was normal, with no evidence of appendicitis as was suspected clinically. The patient was started empirically on IV antibiotics and admitted for observation. On the first night of admission, the patient developed a severe headache, with subsequent rapid deterioration of his mental status. Status epilepticus soon followed. He was intubated for airway protection and transferred to the Medical Intensive Care Unit. A non-contrast CT scan of the head showed diffuse cerebral edema and effacement of the basilar cisterns. Brain MRI performed shortly thereafter revealed abnormal T2 and FLAIR signal hyperintensity involving the bilateral thalami in a symmetric manner, as well as the left external capsule and tegmentum pons (Figures 1, 2, and 3; white arrows). Prominent perivascular congestion was also noted (Figure 4; white arrows). Matthew Ethier, MD, and Jeffrey Rogg, MD Images In Medicine eastern equine encephalitis: mRi Findings in two Patients

MRI and encephalography in fatal eastern equine encephalitis.
Eastern equine encephalitis (EEE) is the most severe of the mosquito-borne encephalitides, with a case fatality rate estimated at 36%. We describe 2 fatal cases of seropositive EEE in men. In both patients, EEG monitoring revealed periodic 0.25to 0.5-Hz transients with lower voltages (;20–40 mV) than typically seen in herpes encephalitis (;100–500 mV), and MRI revealed a symmetric pattern of T2...
متن کاملOcular histopathology in Eastern equine encephalitis: A case report
Purpose To describe the ophthalmic symptoms and histopathological findings in a human case of Eastern equine encephalitis (EEE). Observations The patient was a septuagenarian male whose presentation and clinical course were thought to be most consistent with viral meningoencephalitis. ELISA suggested recent infection with EEE virus. Microscopic analysis of the brain demonstrated perivascular ...

Eastern equine encephalomyelitis (EEE), western equine encephalomyelitis (WEE), and Venezuelan equine encephalomyelitis (VEE) viruses are mosquito-borne pathogens that can cause nonspecific illnesses and encephalitis in equids (horses, mules, burros, donkeys and zebras) and humans in the Americas. Some of these viruses also affect birds and occasionally other mammals. No specific treatment is a...
متن کاملComplete Coding Sequences of Eastern Equine Encephalitis Virus and Venezuelan Equine Encephalitis Virus Strains Isolated from Human Cases
We obtained the complete coding genome of an eastern equine encephalitis virus (EEEV) strain, EEEV V105-00210, and the complete genome of a Venezuelan equine encephalitis virus (VEEV) strain, VEEV INH-9813. They were obtained from human cases and are proposed as reference challenge strains for vaccine and therapeutic development in animal models.
